Individual Appointments
At Integrate we strive to make quality healthcare affordable. We understand that a membership may not be the most cost effective choice for everyone and you DO NOT need to be a member to be seen. Individual appointments are available at one flat rate per visit.

Insurance
At this time INTEGRATE is no longer accepting any form of insurance from new patients. As described in the About Us section, INTEGRATE is a membership driven D.P.C. practice. This means no services rendered at our facility can be billed to insurance. It is highly encouraged for our members to also carry insurance with our membership. In particular, plans with high deductibles and or catastrophic coverage match best.
Also, It is important to note that a membership to INTEGRATE is not, in any form, a type of insurance. Nor is it a substitute for insurance. Membership fees only cover services rendered at an INTEGRATE facility.
